1) Is there a way to create a custom menu item (say, in the Images menu) that points to a customized set of images?
2) If so, what formats are acceptable? (pdf, jpg, etc)
3) If not, is there an alternate way to add images? (pdf, jpg, etc.)

It is technically possible to do it by adding folders in the Program Files\SwordSearcher\Images folder and putting jpgs and pngs there, but this is not an officially supported thing since that requires you to treat the program files area as a writable location.

The supported thing to do is to add images to your user books and commentaries with the editor, but at this time, that does not make the images appear in the catalog of images on the main menu.

It's unlikely anything would be overwritten or that an update would break what you add there. Just be aware that adding images to the image store is not an "officially supported" thing and it is possible the way those images are store could change. I don't have any plans to change it at this time so probably not something worth worrying about too much.
